New Patient Registration
You may register as a patient if you live within the inner practice boundary (and if the practice list is open) by completing a new patient registration form and health questionnaire. These are available from reception and can also be downloaded from the above “Registering Online” page.
If you have a chronic health problem such as diabetes, breathing problem, stroke, or heart disease please make an appointment to see a doctor or the Practice nurse.
If you are in good health and are not on any regular medication you will not need to see anyone before making an appointment in the usual way.
At your first appointment the doctor/nurse will take some basic measurements such as height, weight and blood pressure.
All new patients needing repeat medication will need to see a doctor.
The Practice Area
The practice area covers Beswick, Bradford to Briscoe Lane, Clayton, Droylsden up to Moorside Street, Audenshaw up to Lumb Lane, Abbey Hey, Gorton to Gorton Lane. Please ask and we will check your postcode to see if it is in the practice area.
Moving House?
If you move house but stay within the outer practice boundary you may be able to stay on as a patient. This will be at the discretion of the GP Partners.
